Founded in
October 2014, the Department of Critical Care Medicine of The Fourth Affiliated
Hospital Zhejiang University School of Medicine is equipped with 100,000-grade
clean lamina flow airtight rooms and 29 beds. At present, there are 20 beds
available for patients. The department was set up backed by the Department of
Critical Care Medicine of The First Affiliated Hospital, Zhejiang University
School of Medicine, a key specialty of China. Providing strong backing for the
diagnosis and treatment center of critically ill patients and clinical
departments of the hospital, the department has so far carried out various
advanced life support technologies, including respiratory and circulatory
support, enteral and parenteral nutrition support, central venous catheterization,
PICCO monitoring, arterial blood gas analysis, sputum suctioning under
fiberoptic bronchoscopy, analgesia and sedation, continuous renal replacement
therapy, bedside ultrasound, intra-aortic balloon pump, artificial liver and
other technologies. The department provides high-quality rescue and advanced
life support by well-trained staff for patients with disorders in one or more
organs, or system functions, patients with life-threatening, or potentially
high-risk factors caused by different causes.
Now, the
department has () members, with those aged below 35 accounting for ()%,
including 1 senior title, two with intermediate title, 9 masters and ()
undergraduates, making a vigorous team. Here, we race against time to provide
the most efficient services for patients. In order to better save life and
ameliorate patients' pain, all of us adhere to the "people-oriented"
concept and take care of patients with love, patience and responsibility while
avoiding any error. We are a team with good unity and cohesion.
In order to
further study and implement the spirit of the 19th National Congress
of the CPC and President Xi Jinping thought on socialism with Chinese
characteristics in the new era and carry out the spirit of the 14th Party Congress of the university, the department has, under the leadership of
the Party Committee of the hospital and taking the opportunity of
"Building Youth Civilization", guided young party members and league
members to play a vital role, promoted a high-level occupational civilization
and created first-class achievements. Further, the department has set up a
leading group with director Su Qun as the head and head nurse Zhang Jianchun as
the chief bugler by election in a bid to establish the concept of being medical
quality-centered and fully exert the team culture of "letting life no
longer be alone". Our slogan is (). At the same time, the department
incorporates creation activities into the routine work, holds mobilization
meetings and makes creation plans so that all staff can devote themselves
wholeheartedly to the creation of a youth civilization, fulfill their duties,
encourage each other, set clear goals and clarify ideas.
I. Be
quality-oriented and provide excellent medical services.
Over the past
three years since setup, the department has seen a gradual increase in the need
for beds and by now has received and treated () critically ill patients, with
the cure rate as high as 93%, taking an advanced position in China. In the
clinical diagnosis and treatment of a variety of difficult and severe diseases,
the department is particularly skilled in the diagnosis and treatment of
obstetric critical patients, patients with multiple injuries, patients with
severe pancreatitis and patients with severe septicopyemia. By now, the
department has cured and saved () patients with severe pancreatitis, () with
multiple injuries and nearly 200 patients with
septicopyemia. In July 2017, we successfully treated a lying-in woman
with severe amniotic fluid embolism transferred from Yiwu Maternal & Child
Health Care Hospital. When she was admitted to the hospital, she showed
disseminated intravascular coagulation, extremely unstable circulation and
failure of multiple organs. In view of her dangerous condition, we gave her a
maximal dose of vasoactive drugs to maintain blood pressure and applied the
respirator to give pure oxygen to maintain her respiratory function. To stop
continuous bleeding from incisions and puncture points, we applied
anti-infection, bedside renal replacement therapy, a large amount of blood
transfusion, internal environment disturbance correction and other active
treatment. As a result, the patient was out of danger and discharged on August
4. Centering on "medical quality", we have strengthened the specifications
for medical reports, paid close attention to the quality of documents, strictly
implemented various technical operation norms, effectively prevented medical
errors, further enhanced first aid skills training and improved the ability to
diagnose and treat all kinds of critically ill patients. As a result, we have
provided systematic and quality medical services for residents in Yiwu and even
in middle Zhejiang with assured security and effectiveness of medical
behaviors.
II. Attach
great importance to talents and build the best team structure.
Upholding the
principle of "making good use of existing talents, introducing urgently
needed talents, motivating outstanding talents and training future
talents", we have intensified our efforts in the cultivation and introduction
of talents. We have gradually cultivated the clinical skills of technicians at
all levels, made standardized assessments, appointed young and middle-aged
cadres to study abroad, embraced new ideas and worked hard to build
world-renowned specialties. Besides, we have taken the initiative to organize
doctors and physicians below the intermediate level to attend major domestic
conferences at least once a year and those above the sub-senior level to take
part in major international conferences at least once a year. In recent years,
members of the our department have attended 11 international academic
conferences, 52 national academic conferences and 128 provincial academic
conferences and foreign lectures. We hold clinical study and discussion of stubborn
diseases every two weeks.
III. Devote to
scientific research and promote the sustainable development of the discipline.
We have made efforts to create a youth civilization in a bid to stimulate
the innovation consciousness of young medical staff and improve their
innovation ability. Strategically, the combination of clinical work with scientific
research is an essential measure for innovative medical technologies and
programs. Only by constantly developing new technologies can we meet the
growing demand of the people for medical resources and cope with new diseases
and problems, which holds great significance to the sustainable development of
the discipline. The department conducts regular scientific research meetings
every month to keep in line with the development frontier and hot spots of the
discipline and comprehensively improve clinical skills and scientific research
innovation and cooperation capabilities. At present, the department is
undertaking the monitoring gastric emptying in neurocritical patients, BIS
monitoring of the consciousness state in neurocritical patients, has introduced
bedside ultrasound for disease assessment, promoted the research on the
"improvement of prognosis and life quality in critically ill patients with
ultra-early rehabilitation interventions" and published 7 papers in
B-grade academic journals. The department strengthens routine guidance and
teaching, undertakes large-scale popular science education activities and
strives to improve the level of awareness of local patients on CPR.
IV. Build a
warm nursing team based on humanity.
Integrating
meticulous basic nursing and humanistic care, we provide patients with
continuous, friendly, highly safe and highly standard medical care. We carry
out morning question meeting twice a week and business learning or nursing
round monthly. We have finished the bedside comprehensive ability assessment of
advanced nurses and independent induction training assessment of new nurses and
also completed the BLS and ACLS assessments of all nurses. We have appointed 16
people to participate in provincial continuing education classes and set up a
professional CRRT team. We expand our nursing safety quality control team
headed by head nurses and conduct regular quality seminars. All employees are
required to participate in the continuous quality improvement process, get
involved in the collection of data, finish the registration and reporting of
relevant forms and finish 1-2 CQI projects per year. Besides, we have
intensified monitoring over ICU specialist nursing sensitive indicator. We have
developed an innovative shift mode, focused on the training of nurses' SBAR
communication mode, practically implemented details and thus gained
satisfactory outcomes. The head nurse Zhang Jianchun has initiated the concept
of multi-team cooperation in early activities of ICU patients, combined with
the early intervention of the Rehabilitation Department and made daily rounds,
assessment and assisted patients in need in early rehabilitation exercise.
We have insisted
on building a warm ICU to provide a platform for supportive interactions
between patients and their families. We have also created a warm and festive
environment. When China's Spring Festival comes, we would stick grilles bearing
blessings on ceilings to make bedridden patients see them after sleep. We
concern patients' sleep, pay attention to the role of ICU patients' family
members in the course of ICU patients' disease and utilize the
"family-centered" model to understand patients' family members' needs
and satisfaction to better promote the recovery of ICU patients. In addition to
nursing service satisfactory rate survey of ICU patients, we also make return
visits to discharged patients and provide continued care.
We have a ward
rescue team that has rescued and saved 19 patients who suffered cardiac arrest
in the ward. We have also actively participated in Yiwu Marathon Medical
Volunteer Service every year and "Smile Action" volunteer activities
throughout the country. We're always on the front line out of our passions for
our work.
V. Grasp the
youth and shoulder social missions.
Apart from our
busy work, we have led a colorful sparetime life. In the department's
demonstration classroom, we have built a book corner, enriched our knowledge
and broadened our vision through exchanging books and self-service borrowing;
we have actively participated in the hospital's annual fun games and welcome
parties, demonstrating our passion and vitality of youth. We carry out trade
union activities twice a year to enhance emotion among our colleagues.
Resting on our
profession, we actively exhibit the vitality of young people and bear social
missions. 2017 marked the 120th anniversary of the founding of
Zhejiang University. The department took part in series of activities to
celebrate the anniversary on behalf of the hospital and demonstrated the
outcomes of ICU humanistic care, which was broadcasted at ZJSTV Qianjiang
Channel; we made a poster of ICU nursing team, shot ICU characteristic mission
video and put them into use; we also gave news reports on the intranet of the
hospital and published a total of () press releases. In the "Tencent 9.9
Public Welfare Day Donation Activity", a donation activity was initiated
by nurse Wu Qingqing in the hospital and in the end, 21,350 yuan was raised for
the Mother's Smile Action. As ICU doctors and nurses, we may be happy, sad or
tired. But, in the face of critically ill patients, we are more fearless and
calm, for we are a team. Our efforts have also been affirmed by patients, and
pennants on the wall have witnessed our joint efforts against death for life.
As young
medical workers in the new era, we actively respond to and implement the spirit
of the 19th National Congress of the CPC. The Support Branch for
Critically Ill Patients has given lectures through PPT, held group discussions
to share respective experience and learn, connected with grass-roots branches
and carried out gratuitous diagnosis activities.
Now, we will, taking
the opportunity to create a youth civilization, motivate all young employees,
always adhere to the "people-oriented" principle, focus on the
quality of medical care and medical culture construction and establish a
standard, homogenous and humanistic medical care service team in an effort to
take the lead in middle Zhejiang by integrating medical service, education and
research and make contributions to creating a youth civilization in the new
era.
Introduction to the Department features
The Department of Hematology Specialist Clinic is open from Monday to Sunday. In addition, a sub-specialty clinic for four major diseases of the blood is added every week, including chronic myeloid leukemia/myeloproliferative oncology clinic, acute leukemia/MDS specialist clinic, multiple myeloma/lymphoma specialist clinic, and anemia specialist clinic.
At the medical technology level, it has advanced diagnostic equipment such as bone marrow puncture chamber, bone marrow cell morphology diagnosis room, and blood cell separator. It also relies on the flow cytometry of Tianjin Xiehe Institute of Hematology, which ranks first in the field of hematology, and advanced medical equipment such as real-time quantitative PCR, immunosolid phase electrophoresis, and FISH. Using advanced experimental techniques such as molecular biology, cell and molecular genetics, and immunology, the diagnostic accuracy rate is at the leading level in China.
